Graduate Application Checklist:
The Department of Health Policy and Administration uses a "rolling" application cycle. Applicants are welcome to submit their materials at any time throughout the year for the next-occurring term entry date. However, complete application packets must be submitted no later than six weeks prior to the course start date for timely processing.
A complete graduate application includes:
1. Completed Application and $75 Application Fee. Online application.2. Letter of Intent and Introduction sent directly to the department.
3. Three Original Letters of Recommendation sent directly to the department.
- Letters of recommendation can be from professors, supervisors, co-workers, or other mentors.
- Letters of recommendation must be dated within the past 12 months.
4. Official Transcripts sent to the Graduate School. Official transcripts are those sent directly from the registrar to the Graduate School. Transcripts required include:
- Colleges or universities from which any degrees have been granted or are expected and those transcripts which show the last 60 graded semester or 90 graded quarter hours of undergraduate work taken.
- Colleges or universities showing graded graduate level (including doctoral) coursework taken after the bachelor's degree.
5. Official GRE or GMAT Scores sent to the Graduate School.
- The GRE or GMAT tests must have been taken within the past five years.
- When taking the GRE or GMAT test, indicate Washington State University as a score recipient.
- WSU school code is 4705; there is no department code. If you did not indicate WSU when you took the test, you will need to contact ETS to request a score report be sent to WSU.

6. Official TOEFL Score (for international applicants).

7. One-page Resume
- Please be sure to limit your resume to one-page.
- Include relevant information regarding work or volunteering related to the healthcare industry, administrative experience, or business experience.
