Instructions

1. Please contact us to get a quote for your project.
2. Send us a purchase order.
3. Download and complete the Sample-Submission-Form and User-Genomics-Core-Agreement.
4. Complete the form below with Name, Date, Email, and upload your completed copy of the Sample Submission Form. Print a hard copy of the completed Sample Submission form to be mailed in with your samples. Proceed to the bullet points below the form.

Pack your samples with blue ice or dry ice. Blue ice is required for DNA samples and user-made libraries; dry ice is required for RNA samples.
Mail the hard copies of the Sample-Submission-Form, User-Genomics-Core-Agreement, and your samples via overnight delivery to the following address:
